From there, re-open the game, and your old save file should still be there, but it won't be stuck on Safe mode (but you'll still have the trophy. Once you have it, immediately quit out of the game without saving. You won't be able to do much damage to him, but on this difficulty, if your party dies, you come back to life with all your HP and SP restored, but the Reaper doesn't get any health back. Once you find him, let him ambush you (I've been told this prevents him from doing a certain attack, though I can't confirm this). Once you hear chains, he'll have spawned in (follow the red dots on the map to find him). Once that's done, change the difficulty to "Safe Mode," go to the middle of any floor in Mementos you like (I picked an early one), and sit still for a couple minutes. At this point, I'd recommend making a backup of your save file on a USB drive on the off-chance something goes wrong (If you don't know how to do this, follow these instructions. Once you think you're ready to take him on, head to Mementos and save your game. I would also recommend bringing three party members with maxed-out confidants (so they'll have have their upgraded Personas). I would recommend waiting until the late-game to attempt this, when your party is around level 70 (it can technically be done as soon as you gain access to Mementos, but it will take much longer, as you'll be much weaker). It will usually cast concentrate on one turn and then almighty on the next. This trophy may seem a little intimidating at first (especially since the trick to defeating him in the vanilla game no longer works), but thankfully there is a simple trick that makes this trivial. This will reduce its actions from 2 to 1 per turn. Please note that this is NOT a guide to defeat this boss legitimately, this is simply a way to get the trophy with minimal trouble and without permanently locking your save file on Safe Mode. On "Safe Mode," this isn't that big a deal, but if you're playing on any other difficulty (even Easy), he's quite a challenge. He has a lot of deadly attacks (some of which can one-shot you on Normal difficulty), lots of health, and insanely high defense, meaning your attacks won't do much damage. For the uninitiated, the Reaper is a really tough enemy that appears in Mementos if you stay still for a long time (around 3-4 minutes or so).
